استسقاء الكليةUrinary disease

انتفاخ حوض الكلية بالبول لأن طريق خروجه مسدود، غالباً بحصوة أو تضخم بروستاتا أو ورم. لو طال الانسداد تتضرر الكلية.Swelling of the kidney’s collecting system with urine because its outflow is blocked, usually by a stone, an enlarged prostate or a tumour. Prolonged blockage damages the kidney.

مصفاة الدمThe blood filter

الكليةThe kidneyفيها حوالي مليون وحدة تصفية (نيفرون) تنظف الدم وتطلع البول.About a million filtering units (nephrons) clean the blood and make urine.
الحالب والمثانةUreters and bladderالحالب ينقل البول للمثانة، والمثانة تخزنه لين يطلع من الإحليل.The ureters carry urine to the bladder, which stores it until it leaves via the urethra.

الأسبابCauses

شي يسد الطريقSomething blocks the way

جهة واحدةOne sideحصوة في الحالب أو ورم أو تضيق.A ureteric stone, tumour or stricture.
الجهتينBoth sidesتضخم البروستاتا أو انسداد عنق المثانة.Enlarged prostate or bladder-outlet obstruction.

وين الخلل؟Where is the defect?

ضغط راجع على الكليةBack-pressure on the kidney

الانتفاخDilationحوض الكلية والكؤوس تتوسع بالبول المحبوس.The pelvis and calyces dilate with trapped urine.
الخطر InfectionDangerالبول الراكد يلتهب، والانسداد مع حرارة طارئ.Stagnant urine gets infected; obstruction with fever is an emergency.

العلاجTreatment

نصرّف البولDrain the urine

التصريفDrainageقسطرة بولية، أو دعامة حالب، أو أنبوب من الظهر للكلية.A urinary catheter, ureteric stent or nephrostomy tube.
السببThe causeإزالة الحصوة أو علاج البروستاتا أو الورم.Remove the stone or treat the prostate or tumour.

وش يصير للمريض؟What happens to the patient?

الأعراضSymptoms

ألم خاصرةFlank painمستمر أو مغص.Constant or colicky.
قلة بولLow urine outputخاصة لو الجهتين.Especially if both sides.
حرارة ورجفةFever and rigorsلو التهب البول.If urine becomes infected.
ارتفاع الكرياتينينRising creatinineعلامة ضرر الكلية.A sign of kidney damage.

الألتراساوند هو الفحص الأول.Ultrasound is the first test.

لمعرفة الضرر.To gauge damage.

Obstruction + feverلو سألك الاختبار: حصوة مع انسداد وحرارة ورجفة = تصريف عاجل ومضاد حيوي، مو مجرد مسكن.If the exam asks: an obstructing stone with fever and rigors = urgent drainage and antibiotics, not just analgesia.

احفظ: استسقاء الكلية مو مرض بحد ذاته، هو علامة إن فيه انسداد لازم ينفتح.Remember: hydronephrosis is a sign of obstruction that must be relieved, not a disease by itself.
