سلس البولUrinary disease

تسرب البول بدون إرادة. أشهر أنواعه الجهدي (مع الكحة والضحك) والإلحاحي (رغبة مفاجئة قوية). شائع عند النساء وكبار السن ويؤثر على الحياة.Involuntary leakage of urine. The main types are stress (with coughing or laughing) and urge (a sudden strong need). Common in women and older adults, affecting quality of life.

مصفاة الدمThe blood filter

الكليةThe kidneyفيها حوالي مليون وحدة تصفية (نيفرون) تنظف الدم وتطلع البول.About a million filtering units (nephrons) clean the blood and make urine.
الحالب والمثانةUreters and bladderالحالب ينقل البول للمثانة، والمثانة تخزنه لين يطلع من الإحليل.The ureters carry urine to the bladder, which stores it until it leaves via the urethra.

ليش يتسرب؟Why does it leak?

الجهدي StressStressضعف قاع الحوض، فيتسرب مع الكحة والعطس.Weak pelvic floor; leaks with coughing and sneezing.
الإلحاحي UrgeUrgeعضلة المثانة تنقبض فجأة، فما يلحق الحمام.The bladder muscle contracts suddenly, so he cannot reach the toilet.

أنواع أخرىOther types

الفيضان والوظيفيOverflow and functional

الفيضان OverflowOverflowمثانة ممتلئة ما تفضى، تقطير مستمر، مثل تضخم البروستاتا.A full bladder that will not empty, with constant dribbling, as in BPH.
الوظيفيFunctionalالمثانة سليمة لكن ما يقدر يوصل الحمام: حركة أو خرف.The bladder is fine but he cannot reach the toilet: mobility or dementia.

Stress vs urgeلو سألك الاختبار: تسرب مع الكحة = جهدي (كيجل)، ورغبة مفاجئة ما يلحق = إلحاحي (تدريب مثانة).If the exam asks: leakage on coughing = stress (Kegels); sudden urge he cannot hold = urge (bladder training).

احفظ: لا تقلل الماء كثير عند السلس، البول المركز يهيج المثانة.Remember: do not restrict water too much in incontinence; concentrated urine irritates the bladder.
