ما هو؟
سرطان في نخاع العظم ينتج خلايا بيضاء غير ناضجة تمنع صنع الخلايا الطبيعية.
الأعراض
تعب وشحوب، وحرارة وعدوى متكررة، وكدمات ونزيف، وآلام عظام، وتضخم الغدد أو الطحال.
العلاج والتعامل
علاج كيميائي مكثف، وأحياناً زراعة نخاع، مع وقاية من العدوى ومتلازمة تحلل الورم.

التعريف
خلايا سرطانية غير ناضجة (بلاست) تملأ النخاع، فيقل الدم الطبيعي.
الأنواع
- الليمفاوي الحاد (ALL): أشيع سرطان عند الأطفال، ونسبة الشفاء عالية
- النقوي الحاد (AML): أشيع عند البالغين
الأعراض
- أنيميا: تعب وشحوب
- نقص العدلات: حرارة وعدوى
- نقص الصفائح: كدمات ونزيف
- آلام العظام، وتضخم الغدد والطحال
التشخيص
صورة الدم، وخزعة النخاع.
العلاج
علاج كيميائي على مراحل، وزراعة نخاع لبعض الحالات.
الدور التمريضي
- الحرارة مع نقص العدلات طارئ: مضاد حيوي خلال ساعة
- غسل اليدين، وتجنب الخضار النيئة حسب البروتوكول
- سوائل وألوبيورينول لمنع تحلل الورم
نقطة للاختبار
الحرارة ٣٨ فأكثر عند مريض كيماوي تُعامل كإنتان حتى يثبت العكس.

What is it? A bone marrow cancer producing immature white cells that crowd out normal cell production.
Symptoms: Fatigue and pallor, fever and recurrent infection, bruising and bleeding, bone pain, lymph node or spleen enlargement.
Management: Intensive chemotherapy, sometimes stem cell transplant, with infection and tumour lysis prevention.
Definition
Immature malignant cells (blasts) fill the marrow, reducing normal blood cells.
Types
- ALL: the commonest childhood cancer, with high cure rates
- AML: commoner in adults
Symptoms
- Anaemia: fatigue and pallor
- Neutropenia: fever and infection
- Thrombocytopenia: bruising and bleeding
- Bone pain, lymph node and spleen enlargement
Diagnosis
Blood film and bone marrow biopsy.
Treatment
Phased chemotherapy and stem cell transplant for some.
Nursing role
- Fever with neutropenia is an emergency: antibiotics within an hour
- Hand hygiene and dietary precautions per protocol
- Fluids and allopurinol to prevent tumour lysis
Exam point
A temperature of 38 °C or more on chemotherapy is treated as sepsis until proven otherwise.
