Blood and cancer disease

The marrow makes too many red cells because of a mutation (usually JAK2), so the blood thickens and clots more easily. A chronic condition managed with venesection and drugs.

Anatomy

The blood factory

Makes red cells that carry oxygen, white cells that fight infection and platelets that stop bleeding.
An iron-containing protein in red cells that carries oxygen.

السببThe cause

A factory that will not stop

Drives the marrow to make cells without need.
Secondary polycythaemia comes from low oxygen, such as smoking or lung disease.

Where is the defect?

Thick blood

Headache, dizziness, a red face and itching after warm baths.
Clots in heart, brain and veins, and an enlarged spleen.

Treatment

Thin the blood

Regular blood removal to keep haematocrit under 45%.
Low-dose aspirin, and hydroxyurea for high risk.

Diagnosis

Blood count

High haemoglobin and haematocrit.

Raised, often with high platelets and white cells.

JAK2JAK2 test

Positive in most cases.

Low, unlike secondary polycythaemia.

If the exam asks: a red face, itching after a hot shower and haemoglobin 19 = polycythaemia vera.
