مرض فون ويلبراندBlood and cancer disease

أشهر اضطراب نزيف وراثي، فيه نقص أو خلل في عامل فون ويلبراند اللي يلصق الصفائح بجدار الوعاء المجروح. يسبب نزيف أنف ولثة ودورة غزيرة.The commonest inherited bleeding disorder: von Willebrand factor, which sticks platelets to an injured vessel wall, is low or faulty. It causes nose and gum bleeds and heavy periods.

مصنع الدمThe blood factory

نخاع العظمBone marrowيصنع كريات حمراء تنقل الأكسجين، وبيضاء تحارب العدوى، وصفائح توقف النزيف.Makes red cells that carry oxygen, white cells that fight infection and platelets that stop bleeding.
الهيموغلوبينHaemoglobinبروتين بالكرية الحمراء يحتاج حديد ويحمل الأكسجين.An iron-containing protein in red cells that carries oxygen.

السببThe cause

خلل وراثيAn inherited fault

الوراثةInheritanceغالباً من أحد الوالدين، ويصيب الرجال والنساء.Usually from one parent, affecting men and women.
الوظيفةFunctionالعامل يلصق الصفائح ويحمي عامل التخثر الثامن.The factor sticks platelets and protects factor VIII.

وين الخلل؟Where is the defect?

سدادة ضعيفةA weak plug

الأغشيةMucous membranesنزيف أنف ولثة، ونزيف بعد قلع الأسنان.Nose and gum bleeds, and bleeding after dental extraction.
النساء MenorrhagiaWomenدورة غزيرة طويلة، ونزيف بعد الولادة.Heavy, prolonged periods and postpartum bleeding.

العلاجTreatment

نرفع العاملRaise the factor

ديزموبريسين DDAVPDesmopressinيطلق العامل المخزون، قبل الإجراءات البسيطة.Releases stored factor, before minor procedures.
المركز والمساعدConcentrate and supportمركز العامل للنزيف الكبير، وترانيكساميك أسيد للأغشية.Factor concentrate for major bleeds; tranexamic acid for mucosal bleeding.

وش يصير للمريض؟What happens to the patient?

الأعراضSymptoms

نزيف أنف متكررFrequent nosebleedsمن الطفولة.Since childhood.
كدمات بسهولةEasy bruisingبدون إصابة كبيرة.Without major injury.
دورة غزيرةHeavy periodsمن أول دورة.From the first period.
نزيف بعد الأسنانBleeding after dental workأو العمليات.Or surgery.

التشخيصDiagnosis

تحاليل التخثرClotting tests

الصفائح طبيعية والعامل منخفض.Normal platelet count, low factor.

١1عامل فون ويلبراند VWF antigenVWF level

منخفض أو لا يعمل.Low or not working.

٢2العامل الثامن Factor VIIIFactor VIII

قد يكون منخفضاً.May be low.

٣3الصفائح Platelet countPlatelets

عددها طبيعي غالباً.Usually a normal count.

Mucosal bleeding + normal plateletsلو سألك الاختبار: بنت عندها نزيف أنف متكرر ودورة غزيرة والصفائح طبيعية = فون ويلبراند.If the exam asks: a girl with recurrent nosebleeds, heavy periods and a normal platelet count = von Willebrand disease.
