فتق الحجاب الحاجزDigestive disease

جزء من المعدة يطلع من فتحة المريء في الحجاب الحاجز إلى الصدر، فيضعف الصمام ويزيد الارتجاع.Part of the stomach pushes up through the oesophageal opening of the diaphragm into the chest, weakening the valve and worsening reflux.

من المريء للمستقيمOesophagus to rectumالأكل ينزل من المريء للمعدة، ثم الأمعاء الدقيقة تمتص الغذاء، والقولون يمتص الماء.Food passes from the oesophagus to the stomach; the small bowel absorbs nutrients and the colon absorbs water.

جانبي RollingRolling (paraoesophageal)جزء من المعدة يطلع جنب المريء وقد يختنق.Part of the stomach rolls up beside the oesophagus and may strangulate.

العلاجTreatment

نخفف الضغط والحمضLess pressure, less acid

نمط الحياةLifestyleنقص الوزن، وجبات صغيرة، ورفع رأس السرير.Weight loss, small meals and a raised bed head.

الجراحة FundoplicationSurgeryتلف قمة المعدة حول المريء للحالات الكبيرة.Wraps the top of the stomach around the oesophagus for large hernias.

الأعراضSymptoms

غالباً بدون أعراضOften silentتنكشف صدفة بالأشعة.Found by chance on imaging.

حرقة وارتجاعHeartburn and refluxتزيد بعد الأكل والانحناء.Worse after meals and bending.

امتلاء سريع وألم صدرEarly fullness and chest painخصوصاً في الفتق الجانبي.Especially with a rolling hernia.

ألم شديد وقيءSevere pain and vomitingاختناق الفتق: طارئ.Strangulation: an emergency.

بلع الباريوم أو المنظارBarium swallow or endoscopy

تظهر المعدة فوق الحجاب الحاجز.They show stomach above the diaphragm.
